EHI Export
This document describes Avon's support for ONC §170.315(b)(10) Electronic Health Information export.
Per (b)(10) ONC Certification, Avon supports two types of electronic health information exports:
- Single patient
- System/patient population
EHI Export
What is Electronic Health Information (EHI)?
Electronic Health Information (EHI) refers to electronic protected health information (ePHI) that is part of a designated record set, as defined under HIPAA. This includes:
Medical and billing records about individuals maintained by or for a healthcare provider.
Enrollment, payment, claims adjudication, and case or medical management records maintained by or for a health plan.
Other records used, in whole or in part, to make decisions about individuals.
EHI does not include psychotherapy notes as defined in 45 CFR 164.501 or information compiled in reasonable anticipation of, or for use in, a civil, criminal, or administrative action or proceeding.
In short, EHI covers the same electronic health information that individuals have the right to access under the HIPAA Privacy Rule.
Generating an Export
An admin can initiate a single-patient export by navigating to a patient’s profile and pressing the "Export EHI" button.
The patient population data export is handled by the Avon Support team as it depends on size of data, time and efforts required to manage the server resources. Please email support@avonhealth.com with the subject line “Patient Population b10 Export Request” to initiate the process.
Export Format
The EHI Export process generates a .zip file that includes CSV files, PDF documents, PNG images, and a dedicated folder for each patient.
For the patient population export, the .zip file contains one directory per patient. Each directory is named using the following convention:
<FirstName><LastName>_<MRN>
The contents of each patient directory mirror those of the individual patient export. Further details on the included files are provided below.
Data Categories Included
The export includes all electronic health information (EHI) that is part of the designated record set, organized into the following categories:
Demographics
- Name
- Date of birth
- Sex
- Race and ethnicity
- Language preferences
- Addresses
Clinical Information
- Allergies and adverse reactions
- Medications, including prescription history and active medications
- Problem list (diagnoses)
- Immunizations
- Family history
- Vital signs
- Procedures
- Surgical history
- Lab results
- Imaging results
- Clinical notes (e.g., progress notes, history and physical, discharge summaries)
- Care plans
Administrative and Billing Information
- Appointments
- Insurance details
- Insurance claims
- Payment history
Other Documents
- Forms
- Uploaded documents (PDFs) and images (PNGs)
