Visit Notes
You can create visit notes for patients from customized visit note templates.
All of the steps listed below can also be done using Avon's APIs.
Prerequisites
Part 1: Create a visit note template (EMR)
First, create a customized visit note template.
Follow the instructions here to create your visit note template.
Steps
Step 1: Create a visit note (EMR)
To create a visit note:
- Go to the EMR (e.g. emr.avonhealth.com)
- Click on the hamburger menu on the top left corner > Patients > Click on "View chart" for the appropriate patient and then either:
- Click "Add" and select "Visit Note" from the dropdown OR
- Click the "Visit Notes" shortcut on the top bar and click "Add a new visit note"
- The "Add visit note" modal will pop up. In it, select:
- Patient: The patient should be pre-selected for you.
- Select template: Select a visit note template
- Click "Create Visit Note"
- Fill out the selected template
- Use the AI Scribe to automatically draft the SOAP note sections of the appointment
- All information set is autosaved so click “Finish and Go Back” if you are done with the visit note for now
Step 2: Copy from an existing note (EMR)
Instead of filling out a note from scratch, you can pull the contents of an existing note into the one you are working on.
To copy from an existing note:
- Select the "Previous Note" option ("Copy from an existing note")
- Under "Select previous note", use the dropdown to choose which note you want to copy from. Notes are listed by their template name and date, for example "Dr. Anand New Patient: August 14, 2026"
- Under "Choose what to copy over", all of the copyable fields are listed with their check boxes already checked, grouped by the section they belong to (for example "Subjective" and "Objective"). Uncheck the box of any question whose contents you do not want to bring over, or click "Deselect all" to clear them all and pick only the ones you want
Notes:
- Only fields that have been marked as copyable in the visit note template appear in this list. To control which fields can be copied, edit them in the visit note template.
- Copied contents can still be edited afterwards like any other response.
Step 3: Request review (EMR)
You can request review of the visit note. Follow the instructions here to request review.
Step 4: Add comments (EMR)
You can add comments to the visit note. Follow the instructions here to add comments.
Step 5: Sign and lock (EMR)
You can sign and lock the visit note. Follow the instructions here to sign and lock.
Step 6: Request patient signature (EMR)
You can request a patient to sign the visit note. Follow the instructions here to get a patient's signature.
Step 7: Generate a PDF (EMR)
You can generate a PDF of the visit note. Follow the instructions here to generate a PDF.
Step 8: Fax document (EMR)
You can fax the visit note. Follow the instructions here to fax it.
Step 9: View, edit, and delete visit note (EMR)
To view, edit, or delete visit note:
- Go to the EMR (e.g. emr.avonhealth.com)
- Click on the hamburger menu on the top left corner > Patients
- Click on the “View Chart” button of the appropriate patient
- Either:
- Scroll through the patient’s chart to find the desired visit note OR
- Click the "Visit Notes" shortcut on the top bar and scroll to find the visit note document
- Click on the visit note card to go into viewing mode
- Toggle the top dropdown to go into editing mode
- Scroll to the bottom right-hand corner of the card to find the “Delete” button (NOTE: the “Delete” button is only visible when you are in editing mode). When a visit note is deleted, all tasks within the visit note will also be automatically deleted.
