Requisitions

A requisition is the order form you hand to a lab or imaging center. Use requisitions when you are ordering from a facility Avon does not connect to electronically: Avon builds the requisition from the patient's chart, generates a PDF you can send or print, and tracks the order through to results.


Prerequisites

Part 1: Set up your service facilities (EMR)

Every requisition names the facility performing the service. Follow the instructions here to set up your service facilities.

Part 2: Create your order sets (EMR)

An order set is a saved bundle of orders — the tests you always order together for a given workup. Applying one when you create a requisition pre-populates it so you are not retyping the same panel every time.

To create an order set:

  • Go to the EMR (e.g. emr.avonhealth.com)
  • Click on the hamburger menu on the top left corner > Settings > Modules > Order Sets
  • Click "Create an order set"
  • Give it a name, and optionally a description or instructions
  • Under "Scripts Ordered", add the orders that make up the set
  • Order sets are typed as general, lab, or imaging, and only sets matching the requisition's type are offered when you create one

Steps

Step 1: Create a requisition (EMR)

Where you create a requisition determines its type.

To create a general requisition:

  • Go to the EMR (e.g. emr.avonhealth.com)
  • Click on the hamburger menu on the top left corner > Patients
  • Click on the "View Chart" button of the appropriate patient
  • Click the "Requisitions" shortcut on the top bar
  • Open the "Actions" dropdown and select "Create requisition"
  • Select an order set to pre-populate the requisition, or leave it blank to build it from scratch

To create a lab requisition, do the same from the patient's "Labs" tab and select "Create lab requisition". Follow the instructions here.

To create an imaging requisition, do the same from the patient's "Imaging" tab.

You can also collect requisitions from a patient or a member of your team through a form, using the requisition question type. Follow the instructions here.

Step 2: Fill out the requisition (EMR)

Fill out the requisition:

  • Name: A name for the requisition. Avon defaults this to the type and today's date.
  • Patient: Confirm the patient is correct
  • Ordering Provider: The provider ordering the service
  • Service Facility: The facility performing the service. Follow the instructions here to set these up.
  • Order Details: Any instructions for the performing facility
  • Diagnoses: The ICD codes supporting the order
  • Scripts Ordered: The orders themselves. Each can be linked to the diagnoses that support it.

All information is autosaved, so click "Finish and Go Back" if you are not finished yet.

Step 3: Generate the requisition PDF (EMR)

Click "View Requisition" to generate a PDF of the requisition. Send or print that PDF for the performing facility.

You can also download a requisition from its card in the chart — open the card's menu and click "Download".

Step 4: Share the requisition with the patient (EMR)

If the patient is taking the requisition to the facility themselves, share it with them so it appears in their patient portal.

To share a requisition:

  • Find the requisition's card in the patient's chart
  • Open the card's menu
  • Click "Share with patient"

The card shows "Shared with patient" once it has been shared.

Step 5: Track the requisition (EMR)

Requisition status is set by hand — Avon does not advance it for you. Click the status badge on the requisition's card to change it.

A requisition moves through these statuses:

  • Draft: The requisition has been created but not sent
  • Sent: The requisition has gone to the performing facility
  • Results Received: The results have come back

Step 6: View, edit, and delete requisitions (EMR)

To view, edit, or delete a requisition:

  • Go to the EMR (e.g. emr.avonhealth.com)
  • Click on the hamburger menu on the top left corner > Patients
  • Click on the "View Chart" button of the appropriate patient
  • Either:
    • Scroll through the patient's chart to find the desired requisition OR
    • Click the "Requisitions" shortcut on the top bar to find the desired requisition
  • Click on the requisition card to go into viewing mode
  • Toggle the top dropdown to go into editing mode
  • Scroll to the bottom right-hand corner of the card to find the "Delete" button (NOTE: the "Delete" button is only visible when you are in editing mode)
