Form

You can pre-create forms and then assign forms to patients or complete forms on behalf of patients. Each form can be taken multiple times.

Here are the following types of questions that you can ask in the form:

  • Billing Information
  • Consent
  • Multiple Choice
  • Dropdown
  • Short Answer
  • Grid
  • Number
  • Paragraph
  • Date
  • Time
  • File
  • Statement

All of the steps listed below can also be done using Avon's APIs.


Prerequisites

Part 1: Create a form (EMR)

First, create a customized forms.

Follow the instructions here to create your form.


Steps

Step 1: Complete a form (EMR)

To complete a form:

  • Go to the EMR (e.g. emr.avonhealth.com)
  • Click on the hamburger menu on the top left corner > Patients > Click on "View chart" for the appropriate patient > Click on the "Forms" shortcut on the topbar
  • You'll see a list of all of the available forms
  • Each form card will have:
    • Form Reminders Tables: A table listing all of the times the patient was requested to complete this form. Once the patient has completed the form, the reminder entries will disappear.
    • Form Responses Table: A table listing all of the form responses submitted by the patient or on behalf of the patient for this form. You can click on the "View Responses" button to see the full form response.
    • Status Tag: Status tag that says one of the following:
      • In Progress: This form has a form response that's still in progress (the user started filling out the form but hasn't submitted it yet).
      • Submitted: This form has at least one submitted form response and no form responses in progress.
      • Not Started: This form has no submitted form responses or form responses in progress.
    • If the form is in status "In Progress," you'll see the button to:
      • Complete Form: Continue completing the form
    • If the form is in status "Not Started," you'll see buttons to:
      • Start Form: Start filling out the form on behalf of the patient
      • Request Completion From: Request the patient to complete the form
    • If the form in in status "Submitted," you'll see buttons to:
      • Restart Form: Create a new form response to the form
      • Request Completion From: Request the patient to complete the form
  • Click "Start Form" or "Continue Form" to start/continue responding to a form as appropriate
  • Fill out the requisite information in the form
  • All information set is autosaved so click “Finish and Go Back” if you are done editing the form response for now
  • Click "Submit" to submit the form response. Once a form response is submitted, it can no longer be edited.

Step 2: Complete a form (Patient Portal)

For a patient to complete a form from the patient portal:

  • Go to the patient portal (e.g. portal.avonhealth.com)

  • Click on the hamburger menu on the top left corner > Forms

  • Under the "Uncompleted Forms" tabs, the patient will find all the forms that the organization member has requested them to complete and that the patient hasn't completed yet.

  • Each form card will have:

    • Status Tag: Status tag that says one of the following:
      • In Progress: This form has a form response that's still in progress (the patient started filling out the form but hasn't submitted it yet).
      • Not Started: The patient hasn't started responding to the form yet.
    • Form Responses Table: A table listing all of the form responses submitted by the patient or on behalf of the patient for this form. The patient can click on the "View Responses" button to see the full form response.
    • If the form is in status "In Progress," the patient will see the button to:
      • Complete Form: Continue completing the form
    • If the form is in status "Not Started," the patient will see the button to:
      • Start Form: Start filling out the form
  • Under the "Completed Forms" tab, the patient will find all the forms that the patient has completed in the past

  • Click "Start Form" or "Continue Form" to start/continue responding to a form as appropriate

  • All information set is autosaved

  • Click "Submit" to submit the form response. Once a form response is submitted, it can no longer be edited.
