Health Information Exchange Integration

We partner with Metriport for health information exchange. Through the Metriport integration you can query the national networks for a patient's outside medical records, import those records into their Avon chart, and receive real-time notifications when the patient is admitted, transferred, or discharged.

What the integration enables

• External Records: Query the connected networks for a patient's records and import conditions, medications, allergies, labs, notes, and more into their chart. Follow the instructions here.
• ADT Notifications: Receive real-time admit, discharge, and transfer events for your patients.
• Transition Care: Review patients moving between care settings in an embedded transitions-of-care view.

Prerequisites

Participating in health information exchange requires your organization to be enrolled with the networks, which involves signing the network participation agreements and confirming the facilities and provider identifiers you are enrolling under.

Steps

Your Avon contact will assist you in getting set up with Metriport and enrolled with the networks. If you need any help, please don't hesitate to reach out to your Avon contact.

Once your organization is enrolled, each patient is set up individually the first time you open their External Records tab — no additional configuration is needed from you.
