Question Types
The following question types can be used in forms, visit note templates, care plan templates, document templates, and quizzes. Note that some question types are specific to certain templates (e.g., "Setup Payment" is exclusive to forms).
Common Toggles
These are some common toggles that are available in all/most question types:
- Required: Whether a response to this question is required
- Add to Summary Card: (Only applies to visit note templates, care plan templates, and document templates) Whether this question and its' response should show up on the summary card when an organization member is scanning a patient's chart
- Add to Patient Chart: (Only applicable to some question types) Whether to sync the response from this question to the patient panel that's located on the left-hand side in the patient's chart
Types of Questions
Multiple Choice
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
Multiple choices that are listed vertically that the form responder can select one or more options from (depending on if "Multiple Selection" toggle is turned off or on). Each option can be assigned a score. The total score for the form is calculated by adding together the scores for all the options that were selected.
The following toggles are available:
- Image: Whether to show an image with each option
- Multiple Selection: Whether to allow the form responder to select multiple options
- Horizontal Alignment: Whether to align the options horizontally instead of vertically
- Add to Summary Card: (Only available in visit note templates, care plan templates, and document templates)

Dropdown
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
Multiple choices that are listed in a dropdown format that the form responder can select one option from. Each option can be assigned a score. The total score for the form is calculated by adding together the scores for all the options that were selected.
The following toggles are available:
- Dropdown: Whether to include a description with each option and it's name
- Add to Summary Card: (Only available in visit note templates, care plan templates, and document templates)

Short Answer
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
One line for the form responder to type in their answer. Add a placeholder where it says "type your placeholder here" so form responders can see an example or guidance on what to input in the field.
The following toggles are available:
- Add to Summary Card: (Only available in visit note templates, care plan templates, and document templates)

Grid
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
Click "Add row" to add a row to the grid. Type the name of the row where it says "Row 1", etc. Click "Add column" to add a column to the grid. Type the name of the column where it says "Column 1", etc. At the intersection of the row and column, type the number of points that are assigned to selecting this option. The total score for the form is calculated by adding together the scores for all the options that were selected.

- Multiple Selection: Whether to allow the form responder to select multiple options per row
Table
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
Build a custom table with typed columns and labeled rows:
- Add columns via "Add column" and rename each column header
- Set each column's type: Statement, Text, Number, Date, Dropdown, or Checkbox
- Add rows via "Add row" and label each row
- Remove any column or row with its "x"
- Each cell inherits its column's type, which controls the allowed input: Statement displays fixed text, Text accepts free text, Number accepts numeric values, Date accepts a date, Dropdown gives preset choices, and Checkbox gives a toggle
Number
Available for: forms, visit note templates, care plan templates, document templates, and quizzes

Type in the policies that you want the form responder to agree to. To consent to the policies, the form responder will type in their full name, add an electronic signature, and set today's date,

SSN

- Add to Patient Chart: Whether to sync this SSN to the SSN field in the patient's chart.
- Add to Summary Card: (Only available in visit note templates, care plan templates, and document templates)

Address

Allergy
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
All the allergies that are on file for the patient will be automatically added as responses to this question. The form responder can modify these allergies, add new allergies, or delete allergies. If the "Add to Patient Chart" toggle, these modifications, additions, and deletions will also apply to allergies in the patient's chart and be synced to DoseSpot.
The following toggles are available:
- Add to Patient Chart: Whether to sync these allergies back to the allergies field in the patient's chart.

Medication

All the medications that are on file for the patient will be automatically added as responses to this question. The form responder can modify these medications, add new medications, or delete medications. If the "Add to Patient Chart" toggle, these modifications, additions, and deletions will also apply to medications in the patient's chart and be synced to DoseSpot.

Condition

All the conditions that are on file for the patient will be automatically added as responses to this question. The form responder can modify these conditions, add new conditions, or delete conditions. If the "Add to Patient Chart" toggle is turned on, these modifications, additions, and deletions will also apply to conditions in the patient's chart. These conditions will also automatically be synced to the diagnoses section of the insurance claim.
The following toggles are available:

Vitals
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
Space for the form responder to add the patient's vitals.
If the vital signs were captured earlier rather than during this visit, the form responder can document when they were taken by filling in the "Date Captured" field.
The following toggles are available:
- Add to Patient Chart: Whether to sync these vitals back to the vitals section in the patient's chart.

Family History
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
The Family History question type captures structured family medical history by linking relatives to specific diagnoses, with optional dates and comments. A single question can hold many relatives, and each relative can carry many diagnoses, making it suited for thorough intake where multiple conditions span multiple family members.
Structure
The question is organized into three columns:
- Relationship: A dropdown for selecting the relative, with options including Mother, Father, Sister, Brother, Maternal Grandmother, Maternal Grandfather, Paternal Grandmother, and Paternal Grandfather.
- Diagnoses: A searchable dropdown of ICD-coded diagnoses. Each selected diagnosis can carry its own date.
- Comments: A free-text field for additional context tied to each relative.
How it works
- Select a relationship from the dropdown.
- To assign a condition, search and select an ICD-coded diagnosis (for example, A00.0: Cholera due to Vibrio cholerae 01, biovar cholerae) and set an optional date for it.
- Use + Add Diagnosis to attach more than one diagnosis to the same relative, and remove any individual diagnosis with the red X.
- Enter any notes for that relative in the Comments field.
- To log another relative, use + Add Family History; to remove an entire relative row, use the red trash icon.
Note that dates are tied to each diagnosis rather than to the relative, which allows precise onset or recording tracking when a single relative has several conditions logged on different dates.
Configuration options (builder)
- Scribe Instructions: Add custom guidance for the AI Scribe when it populates this field.
- Logic and Conditions: Conditionally show the question only when defined criteria are met (supports + Or logic for multiple conditions).
- Add to Patient Chart: Pushes the captured data into the patient chart.
- Show on Summary Card: Surfaces it on the patient summary card.
- Required: Makes the question mandatory before the note can be finalized.
Phone Number
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
Space for the form responder to type in a phone number.
The following toggles are available:
- Add to Patient Chart: Whether to sync this phone number to the phone number field in the patient's chart.
- Add to Summary Card: (Only available in visit note templates, care plan templates, and document templates)

Emergency Contacts
Available for: forms, visit note templates, care plan templates, document templates, and quizzes
Space for the form responder to add one or more emergency contacts. For each contact, enter a name, relationship, and phone number. Click "+ Add Emergency Contact" to add another contact, or the minus icon to remove one.
The following toggles are available:
- Add to Patient Chart: Whether to sync these emergency contacts to the emergency contacts field in the patient's chart.
- Add to Summary Card: (Only available in visit note templates, care plan templates, and document templates)

PDF Upload

Upload a PDF for the form responder to view.
Editor

Pre-set default text. Insert variables for information that should be populated with the appropriate patient's information at run-time; for example, “{{patient.first_name}} {{patient.last_name}} is born on {{patient.date_of_birth}} and prefers to speak {{patient.preferred_language}}.”

Set default ICD codes that the form responder can add to or delete from. These ICD codes will be automatically synced to the diagnoses section of the insurance claim.

CPT Codes

Set default CPT codes and modifiers that the form responder can add to or delete from. These CPT codes will be automatically synced to the billing items section of the insurance claim.

Form Response
Available for: visit note templates
Space for the form responder to fill out a form directly inside the visit note, so that the form is documented as having been filled out during that visit.

Task

Creates a task for the form responder to set up. Enter a task title and description, set a priority (Low, Medium, or High), and assign a due date and an assignee. Choose whether to share the task with the patient. The task will be created and routed to the assigned organization member.

Requisitions
Available for: visit note templates, care plan templates, and quizzes
Space for the form responder to create one or more requisition orders. For each requisition, enter a name, select an ordering provider and service facility, and add order details. Add one or more diagnoses (each with active status, onset date, and end date) and one or more scripts ordered, which can be linked to the associated diagnoses. Optionally apply an order set to pre-populate the requisition. Click "+ Add Requisition" to add another requisition.
