Patient Signature
You can collect patient signatures on notes, care plans, and documents to get their attestation that the contents of the object are accurate. The note, care plan, or document can still be edited after the patient signs it.
Get patient signature (EMR)
To get the patient's signature on a note, document, or care plan:
- Go to the EMR (e.g. emr.avonhealth.com)
- Click on the hamburger menu on the top left corner > Patients > Click on "View chart" for the appropriate patient
- Find the note, document, or care plan you want to get the patient's signature on and click on it
- Scroll to the bottom of the object and then click on More > Request patient signature
- A confirmation modal will pop up asking the patient to confirm their name, that they've looked over the note, care plan, or document, and that the contents are accurate
- Have the patient click "Sign {{Note, Document or Care Plan}}"
- Once a note, document, or care plan is signed by the patient:
- A green bar will show up at the bottom of the note, care plan, or document stating who it's been signed by and when
