Sign and Lock
Standard organization members can sign and lock any note, document, or care plan. Once the note, document, or care plan is signed, it can not longer to be edited. Any additional changes must be added via addendums.
Sign and lock a note, document, or care plan (EMR)
To sign and lock an object:
- Go to the EMR (e.g. emr.avonhealth.com)
- Click on the hamburger menu on the top left corner > Patients > Click on "View chart" for the appropriate patient
- Find the note, document, or care plan you want to sign and lock and click on it
- Scroll to the bottom of the object to the "Sign and Lock" button and click on it
- A confirmation modal will pop up; click "Sign and Lock {{Note, Document or Care Plan}}"
- Once a note, document, or care plan is signed and locked:
- It will not longer be editable
- A green bar will show up at the bottom of the note, care plan, or document stating who it's been signed by and when
Add an addendum (EMR)
To add an addendum to a locked note, care plan, or document:
- Go to the EMR (e.g. emr.avonhealth.com)
- Click on the hamburger menu on the top left corner > Patients > Click on "View chart" for the appropriate patient
- Find the locked note, document, or care plan you want to add an addendum to and click on it
- Scroll down below the note, document, or care plan to the section labeled "Addendums"
- Click on "+ Add an addendum" to start a new draft addendum
- Type your addendum in the box that says "Type your new addendum here"
- The draft is autosaved as you type, but it is not part of the record until it is committed
- To discard the draft, click "- Delete"
- Only one draft addendum can be open at a time; the "+ Add an addendum" button reappears once the draft is committed or deleted
- Click on "+ Commit addendum to record" to save the addendum
- Once an addendum is committed to the record:
- It can no longer be edited or deleted
- It is listed in the "Addendums" section with the name of the member who wrote it and the date and time it was created
Mark an addendum as entered in error (EMR)
A committed addendum cannot be edited or deleted. If it was added by mistake, it can be marked as entered in error, which strikes it out while keeping it on the record. Only global admins can see the "Mark Error" button.
To mark an addendum as entered in error:
- Go to the EMR (e.g. emr.avonhealth.com)
- Click on the hamburger menu on the top left corner > Patients > Click on "View chart" for the appropriate patient
- Find the locked note, document, or care plan with the addendum and click on it
- Scroll down to the section labeled "Addendums"
- Click on "Mark Error" next to the committed addendum
- A modal will pop up; type the reason in the box that says "Add reason here"
- Click "Mark entered in error"
- Once an addendum is marked as entered in error:
- It stays in the "Addendums" section, but it is greyed out and its author, date, and text are struck through
- An "Entered in Error" badge shows up next to the author's name
- The "Mark Error" button no longer shows up for that addendum
