Logic

You can embed logic into sections and questions of any form, visit note template, care plan template, document template, or quiz. With logic, you can set conditions that need to be met in order for that section or question to be displayed. The condition statements can be combined together with ANDs and ORs to make more complex criteria.

Embed logic (EMR)

To embed logic:

• Go to the EMR (e.g. emr.avonhealth.com)
• Click on the hamburger menu on the top left corner > Settings > Click on Forms, Visit Note Templates, Care Plan Templates, Document Templates, or Quizzes as appropriate
• Find the form/template/quiz that you want to embed logic into and click on it
• Click on the section or question that you want to embed logic on
• Click the "+ Or" button under the Conditions header
• Click the "+ And" button under the "Or continue if..." header
• Select the question, condition, and value that need to be met for this section/question to be displayed
• Click the "+ And" or "+ Or" buttons as appropriate to add additional condition statements that need to be met
• Click the "-" symbol on the right-hand side of the condition to delete the condition
